Understanding Apicoectomy

Apicoectomy, also known as root-end resection, or periapical surgery, is a minor dental procedure aimed at preserving a tooth that would otherwise require extraction. It entails removing the tip of the tooth's root, known as the apex and can be performed on both pediatric and adult patients.


In a root canal procedure, the dentist accesses the interior of your tooth to eliminate infected pulp. This pulp comprises the soft tissue within the tooth, consisting of nerves, blood vessels, and cells essential for the tooth's health and function. When this tissue becomes infected, a root canal is necessary to eradicate the infection, cleanse the root canals, and seal the tooth to prevent further issues. Conversely, an apicoectomy focuses solely on removing the tip of the tooth's root. It is commonly performed after a root canal treatment has already been conducted on the tooth, and further intervention is needed to address persistent issues or infections that extend to the root tip.

An apicoectomy serves as a means to salvage a tooth and prevent extraction, and there are several reasons why we might suggest this procedure.


Failed root canal treatment: If a root canal treatment fails to heal or if infection persists, an apicoectomy may be necessary to remove the infected tissue.


Residual infection: Sometimes, despite successful root canal treatment, the infection may persist in the surrounding bone or tissue, necessitating an apicoectomy.


Canal anatomy complications: Complex root canal anatomy, such as extra canals or severe curvature, may make it difficult to fully clean and seal during initial treatment, leading to the need for apicoectomy.


Root canal obstruction: Calcified or blocked canals can prevent proper cleaning and sealing during initial treatment, requiring surgical intervention.


Access for retreatment: In cases where retreatment through the existing crown or filling is not feasible or would compromise the tooth's structure, an apicoectomy provides access to the root tip for cleaning and sealing.


Presence of cysts or granulomas: When cysts or granulomas form at the tip of the root, an apicoectomy may be necessary to remove the diseased tissue and promote healing.


Tooth fractures or cracks: Fractures or cracks in the tooth that extend beyond the root may necessitate an apicoectomy to address infection or prevent further damage.

The duration of an apicoectomy generally falls within the range of 30 to 90 minutes, although this timeframe can vary based on several factors. One significant factor influencing the length of the procedure is the location of the tooth being treated and the accessibility of its roots. Teeth located in the front of the mouth or with relatively straightforward root anatomy may typically require less time compared to molars or teeth with complex root structures.


Before undergoing an apicoectomy, thorough preparations are made to ensure the procedure's success and the patient's comfort. This typically involves our endodontist conducting an X-ray examination of the affected tooth to visualize the location of infected roots and assess any surrounding tissue damage. This assessment helps in planning the surgical approach and determining the extent of treatment needed. Just before the surgery, a local anesthetic is administered to numb the area inside the mouth where the procedure will be performed, ensuring that the patient remains pain-free throughout. 


Following these preparatory steps, our skilled endodontists will employ microsurgical instruments to access the damaged tooth root by creating a small incision in the gums. With precision and care, the gum tissue will then be gently moved aside to provide clear access to the root. Using magnification and specialized techniques, any inflamed tissue and infected root tip fragments will be meticulously removed. Typically, only a few millimeters of the root, along with any inflamed or infected tissue, will need to be removed to effectively address the issue. After ensuring thorough cleaning of the root canal, a root-end filling will be precisely placed to seal the root and prevent reinfection. Finally, the incision in the gums will be delicately closed with a few stitches, promoting optimal healing.

Post-Procedure Care

After the procedure, it's common to experience some discomfort or mild swelling as the area heals, which is typical for any surgical intervention. To manage any pain, our team will provide guidance on the best medications and precautions to follow during the healing process. Minor bleeding may occur for 1-2 hours following the procedure, so it's advisable to rest with your head elevated to reduce blood flow. Following our endodontist's guidance, you might consider using over-the-counter pain medication for relief if necessary. 


Additionally, antibiotics may be prescribed as a precautionary measure to prevent postoperative infection. While you should continue brushing and flossing as usual, take care around the treated area to avoid disturbing the stitches. Rinsing your mouth gently with a warm saline solution can help eliminate excess bacteria. You should be able to resume normal activities within a day or two, and the stitches will typically be removed within a week.
